The following documents are necessary for enrollment at South Oldham High School:

_____ Residency Affidavit (9005.03-F only necessary if the family does not pay their own utility bills). 

_____ 25-26 Enrollment Online Registration

_____ Original Birth Certificate

_____ Copy of Social Security Card

_____ Current Immunization/Physical

_____ Kentucky Preventative Health Care Exam (out of state transfers have 30 days to submit this). 

_____Transcript from former school

_____ Athletic Survey

_____Eligible for Free or Reduced Lunch (South Oldham HS Website)

_____ IEP or 504 Documents (if applicable)

_____ Gates Identification (if applicable)
